What to Expect in Your First Visit

Patients are seen by appointment only.  When making an appointment, please describe your problem(s) so that you will be allotted an adequate amount of time. Since Dr. Aubry is a specialist and does not manage general medical problems nor provide routine healthcare, you will need to have a primary physician.

 If your health plan requires a referral, we need a signed approval form from your primary care physician before your appointment. Please notify us 24 hours in advance of your appointment if you will be unable to keep your appointment. If you fail to contact us, you may be subject to a missed appointment fee.

During the first visit, Dr. Aubry obtains a complete medical history, and basic information concerning your problem. Based on this consultation, medication, testing, and/or laboratory work may be recommended. If testing is scheduled, you will be given instructions, and procedures will be explained. Feel free to ask questions at any time.

Because Doctor Aubry has a very full schedule, we ask all patients to arrive by their scheduled appointment time. If you would like to fill out your registration and insurance forms in advance and bring them with you to the office, you can click on the links below. You can either type your information into the form and print it out and sign it or simply print out the form and fill it in as you would normally.
